Hershey Medical Group earns PCMH certification

Penn State Hershey Medical Group has announced that all 13 of its adult primary care sites have been awarded the highest level of Patient-Centered Medical Home certification.


The National Committee for Quality Assurance reviewed the sites. The PCMH concept attempts to shift health care delivery from a reactive approach to a focus on long-term problems and preventative care, with a physician-led team integrating and coordinating care.

The group and its associates, including Penn State College of Medicine, are working to integrate the PCMH concept and examine the best ways to use it.

The college also received one of four nationwide grants from the Agency for Healthcare Research and Quality to develop a new national Primary Care Extension Service. The goal is to spread the PCMH approach in health systems, first within the state and then across the nation.

Penn State Hershey has produced a podcast and two video clips on the PCMH concept.
